If you need dental implants to replace your upper back teeth, you might be surprised if your dentist tells you that you need a sinus lift first. While the name can sound a little intimidating, a sinus lift, also known as sinus elevation, is a very common and highly successful procedure. Its main goal is to rebuild lost bone in your upper jaw, creating a strong, thick foundation for your new dental implants to anchor into.

Why Would You Need a Sinus Lift?

Placing dental implants in the upper jaw comes with a unique set of challenges. Right above your upper back teeth are your maxillary sinuses, which are hollow, air-filled cavities located behind your cheeks. When you lose teeth in this area, your body reacts in two ways:

  • The bone shrinks: Without tooth roots to stimulate the bone, the jawbone naturally begins to dissolve and lose both its height and width.
  • The sinus expands: The air-filled sinus cavity actually expands downward, taking over the space where the bone used to be.

Because of this, the wall of bone separating your mouth from your sinus can become paper-thin, sometimes measuring just 1 to 2 millimeters. Since standard implants need at least 8 to 10 millimeters of bone height to stay secure, placing an implant here without treatment would mean it could push straight into the sinus cavity. This can lead to infections and implant failure. A sinus lift solves this problem by gently lifting the thin sinus membrane back up to its original position and placing bone graft material underneath to create new, solid bone.

The Two Ways We Perform a Sinus Lift

Depending on your current bone level and how much height we need to add, Dr. Nilgün Başar will recommend one of two methods:

1. The Crestal (Internal) Sinus Lift

This is a highly conservative, minimally invasive option. We use it when you already have moderate bone height (around 5 to 7 millimeters) and we only need to gain an extra 1 to 3 millimeters.

Instead of making a separate incision, we access the area directly through the small hole prepared for the dental implant. We use gentle instruments to lift the sinus membrane slightly, slide the bone graft material in, and place the implant right away. Because there are no extra cuts or stitches, you’ll experience very little swelling and a much quicker recovery.

2. The Lateral Window Sinus Lift

We use this traditional method when there is more significant bone loss (less than 4 millimeters of bone remaining) and we need to rebuild a substantial amount of bone.

During this procedure, we make a small, gentle incision in the gum tissue on the side of your jaw (under your cheek). Through a tiny opening in the bone, we carefully lift the delicate sinus membrane upward and fill the space beneath it with bone graft material. We then cover the area with a protective membrane and close the gums with sutures. Because we are rebuilding a larger area, we usually let this graft heal and mature for 4 to 6 months before we place the implants, ensuring they have a rock-solid foundation.

What Recovery Really Feels Like

Even though a sinus lift is a surgical procedure, the vast majority of our patients are pleasantly surprised by how smooth the recovery is. The surgery itself is done under local anesthesia, so you won't feel any pain. If you're feeling anxious, we can also use conscious sedation to help you stay completely relaxed throughout the appointment.

The First 24 to 48 Hours:

  • Swelling and Bruising: Some swelling around the cheek and jaw is normal and peaks around the second or third day. Applying an ice pack to your cheek for 20 minutes on and 20 minutes off during the first 24 hours will significantly reduce swelling.
  • Mild Bleeding: You may experience slight oozing from the surgical site or a minor nosebleed. This is normal and will resolve quickly.
  • Pain Management: Any discomfort is easily managed with prescribed pain relievers and anti-inflammatory medications.

Important Rules for a Smooth Healing Process

To help the sinus membrane heal perfectly and keep the bone graft material exactly where it needs to be, there are a few simple but important rules you’ll need to follow for the first two weeks:

  • Don't blow your nose: Blowing your nose creates pressure inside your sinuses, which can tear the healing membrane or shift the graft. Instead, just gently wipe your nose if needed.
  • Sneeze with your mouth open: If you feel a sneeze coming on, let it out with your mouth open to prevent pressure from building up in your sinus cavity.
  • Skip the straws and spitting: The suction from using a straw or spitting can disturb the blood clot and slow down your healing.
  • Hold off on flying: We advise waiting at least 7 to 10 days after a lateral window sinus lift before taking a flight, as changes in cabin pressure can irritate the sinus area.
